Law Firms and Insurance Companies

You must submit a request for our records in writing on letterhead. Your letter must include:

  • Date, location of the incident and Occurrence Number if known
  • Name of the person involved
  • Details of your request 
  • Authorization form signed by client (you can use our authorization form or your own)
  • Authorization form signed by third party(s) if you are requesting their personal information
    • It is your responsibility to verify consent and confirm identities of third parties. 
    • Authorization forms must be signed and dated within the last 6 months

Please include these details if you have them:

  • Accident number or report number
  • Officers name and badge number

Fees and Payment

Our fees are $47.00 for up to 10 images ($8.00 for additionalphotographs) on DVD, plus HST.

You will be contacted by phone or email once the photos and invoice are ready. Payment will be discussed at that time. Photos will not be released until payment is received. If you have any questions regarding this process please phone the Forensic Identification office at 519-245-1250.

In an effort to protect personal privacy, the Strathroy-Caradoc Police Service (SCPS) is no longer providing copies of the MVCR to involved parties. You can request a copy of this report through the Ministry of Transportation’s (MTO)  website. 

Please note that there may be a delay between the accident date and when the collision report is available on the MTO site.

The Strathroy-Caradoc Police Service is no longer offering the option for Law Firms and Insurance Companies to purchase witness statements.

This information will only be made available through the Freedom of Information process, however all third party information will be redacted from reports unless you provide us with their signed consent.

If you need a copy of the officer's notes or statements from a report, you must submit a freedom of information request.
